Welcome! Please read carefully below before scheduling an appointment.
For New Patients (all ages): To book an assessment, please fill out this New Patient Form, and you will be contacted with booking details.
For Returning Patients (all ages): If you are returning for the same concern, and have been seen within the past year, please book four 30-minute follow-ups, spaced 2 weeks apart. If you are returning with a new concern, or haven’t been seen in over one year, please fill out this New Patient Form, and you will be contacted with booking details.
Follow-ups are recommended to be booked ahead of time because there is a high demand for appointments. This ensures follow-up appointments are scheduled within an appropriate time after the assessment. These can always be cancelled or rescheduled if it is determined that they are not required.
PLEASE NOTE: If you do not see an appointment time that works for you, you are welcome to add yourself to the waitlist at the bottom of the booking page. You will be notified as soon as new openings become available.
